- The oropharyngeal airway must be used when the patient is at risk for airway obstruction, owing to reasons, like relaxed upper airway muscles and blockage of the airway by the tongue. Placement of airway is also indicated when performing a head tilt-chin lift maneuver to open the airway. In this procedure, the patient wont be able breathe properly and requires ventilation. In only unconscious patients, oral airways can be used.
- A nasopharyngeal airway is another kind of device for both, conscious and unconscious patients. Especially for semi-conscious patients, it is used for keeping the airways open. There are cases when airways of semi-conscious patients can be at risk of obstruction, but can not have an oropharyngeal airway placed owing to an intact gag reflex.
- Oral airway insertion can be difficult in patients who clench their jaws. It is where nasopharyngeal airways are used. It is helpful in semi-conscious patients and require nasal-tracheal suctioning frequently.

- Contradictions arise when an oropharyngeal airway is used on a conscious patient who has an intact gag reflex. The patients who have the ability to cough can have a gag reflex and should not be ventilated with an oral airway. Oropharyngeal airway must not be used if the patient has a foreign body obstructing the airway.
- Using a nasopharyngeal airway also has a few contraindications. It can not be used on patients having nasal fractures or an actively bleeding nose. Slight bleeding can occur while inserting the airway, which can be suctioned or wiped away.
- Be gentle when inserting any of the airway device.
- A water soluble lubricant must be used when inserting a nasopharyngeal airway.
- Oral airway must not be forced, as it can lead to trauma to the tongue and lips.
- In case resistance is felt during nasopharyngeal airway insertion, one should stop and try the other nostril.
